The fact that there isn't any red sabers in the duel to me is a strong indication that he won't be Vader until after the lava dip. Vader weilding a blue saber just doesn't fit.

There's the death of Windu--opportunity there. The BEST opportunity, being as that solidifies his turn (this scene is the "Choice" scene that everyone's been talking about. Anakin makes his choice to join AT THAT MOMENT--after all the manipulation Palps laid down on him in the Theater)
I'm thinking that it's not so much a choice to become a Sith as a choice to fully ally himself with Palpatine and his ideals (as AotC has set up for us). The Jedi are traitors, so in essence he is that last Jedi that remained loyal to the Old Republic. Perhaps it helps that Palps has promised him additional powers in the force, demonstrated by his lighting on Windu (or cheating death). But just like the Jedi, he must pass his trial before becoming Sith (whether this is a concious test or not I do not know). His test is the Seperatists, the Jedi Temple, and Obi-Wan. Also Sidious must ensure that Padme is dead (or have Anakin think she's dead) before he can fully turn Anakin. Vader can't exist unless all love is gone.

"THAT name no longer has any meaning, for me."
Lucas won't recycle lines like that or "What is thy bidding, my master" ... he'll come up with some original cool lines for Vader methinks.
Passing up a suited Vader fighting seems like it would be hard to do, given that he took a chance a Yoda in AotC. A suited Vader fighting would be a sure thing you would think, not as much of a risk as what he did with Yoda. I'm not so sure that just because we haven't heard reports of a sequence that there isn't one ... one can only hope anyways.
