I was looking in my Stuff magazine the other day, and saw an ad for the OT DVDs.
YES, I know it is an ad for the DVDs, and I know it seems to be a shot from ESB at the "No, I am your father" scene, but if you look closely, the suit seems different, and very similar to the Episode III version. It kinda seems like it was a publicity shot for Ep3 to replicate that scene and it was edited to that background.
Compare with these:
(top: Episode III Vader) and (bottom: ESB Vader)
You can see that his helmet seems smaller in the ad, boxes on his belt father from the buckle, and his dick sheild seems relatively smaller compared to the original movies. Plus in the ad he has the chain for his cape, while the shot from the movie doesn't, and his helmet is tucked in, while the ESB movie is not.
